Do not forget to bring for your intake the following documents and information:

• Copy your identity card,
• Your card of blood-group,
• Name, addresses, personal phone number,
• Name, addresses, telephone of the person to be prevented in the event of accident,
• Name of the attending physician,
• If you are minor, an authorization to operate will have to be signed by your parents.

The service of the intakes will specify you:

• The amount to be regulated for your lodging;

If the anesthetist prescribed you examinations, want to bring, the day of your hospitalization, all the results. Do not send them by the post office. If you have a processing in progress, fit you with your ordinance and your personal medicines. Provide for your : personal purposes of toilet, night effects… Regulation of the expenses of intervention and stay:
As of your intake you will have to discharge regulations provided for with the estimate:

• A regulation for the anesthetist,
• A regulation for the establishment concerning the hospital expenses (care, meal, specific room and fresh specific)

Your intervention day before:

• You will take a shower with a soap disinfectant ( for example if you are not allergic to iodine).
• The area to be operated could be mowed if the surgeon asked it.

The day of your intervention:

• You will have to remain on an empty stomach as from midnight.
• You will take again a shower with a soap disinfectant.
• You will leave for the , covered (E) only of one special insulation without no , neither jewels, neither prosthesis (denture, glass lenses), neither make-up, nor nail varnish.
